![]() All high school and middle school student-athletes need the following forms to participate in interscholastic athletics: 1) Participation Form - This form notes that the student-athlete has parental or legal guardian permission to participate in a specific sport. This form must be signed by both the parent/legal guardian and the student-athlete. This form needs to be handed in at the beginning of each specific sport's season . 2) Physical Form - A student-athlete must obtain a physical before participating in any interscholastic sport. This physical form can be completed by a family physician. The athletic department sponsors free physicals during before the fall, winter and spring seasons, where the form can also be completed. The physical form remains on file in the athletic office and covers the student-athlete for one calendar year . 3) RIIL Assumption of Risk Forms - TWO of these forms must be completed and notarized. Once these forms are on file in the athletic office, a student-athlete is covered for his/her high school career . **Middle school student-athletes need to complete and have notarized only ONE RIPCOA Assumption of Risk Form . Once this form is on file in the athletic office, a student-athlete is covered for his/her middle school career 4) School Insurance - Student-athletes must purchase the $7.50 "School-Time Only" policy. This policy covers the student-athlete for all interscholastic sports played during the academic year. THIS FORM IS NOT AVAILABLE ONLINE. **Football players must purchase the $9.50 "Interscholastic Football" policy. If football players want to participate in another interscholastic sport during the academic year, the must also purchase the $7.50 "School-Time Only" policy.
