403 B

If you would like to start a new 403 B your first step is to download a Vendor Listing to find out who our current vendors are and the next step is to either contact your Accountant or Financial Advisor and ask them to help you choose a vendor that is right for you.  Once you have chosen your vendor you need to request enrollment paperwork from the vendor (your Accountant or Financial Advisor can do this for you).  If you do not have an Accountant or a Financial Advisor there are phone numbers listed on the Vendor Listing and you can contact the vendors directly and they will help you with the enrollment process.

When the enrollment process has been completed, you will need to download a Salary Reduction Agreement and send it to Susan Cucca.  The Salary Reduction Agreement is an authorization for West Warwick Public Schools to make the payroll deduction that you are requesting and will stay in our payroll files.
